This form is intended to be used for all future Fine Art Exhibits, Art in Public Places Exhibits, and Gallery Store item submissions.
Note: Information on the gallery cards and gallery store labels will appear exactly as submitted on this form. So if the information is entered in all upper case or all lower case that's exactly how it will appear on the gallery card or gallery store label. . Please verify all spelling, titles, medium and prices entered.
All fields with an asterisk * following the field name must be filled out or the form can not be properly submitted.
You must complete this form for each piece of artwork that you are submitting. The form will retain your name after the first submission.
It is very important that you select the proper exhibit that your are submitting artwork for.
Fine art Exhibit Submissions

For fine art exhibits all fields must be completed before submitting the form.

Gallery Store Item Submissions

Start all Titles with a number followed by a dash. Ex. 1-Sun Rising

Do not include a image with your gallery store items.

Do not include the size of your gallery store item.
